You can download them and bring them to your first appointment to expedite the process.
Things to bring to your first appointment:
- Photo ID
- Insurance Card
- Athletic clothing (preferably shorts and t-shirt) to allow proper examination
- Any previous imaging study (MRI, X-Ray, CT scan)
